To participate in a sport, student-athletes must complete a physical examination every 13 months (12 months, with a one-month grace period). When your child has their annual well-child exam, we recommend asking the healthcare provider to complete the KS Physical Evaluation form at that time.
Parents should then upload the completed form to their child’s profile in Mo‘omō‘ali Olakino (EHR) Parent Portal. The KS Physical Evaluation form and instructions for logging in and uploading documents to the EHR Parent Portal are available at www.ksbe.edu/malama-ola/forms.
Additionally, student-athletes must have health insurance to participate.
Mālama Ola is the enterprise division responsible for medical clearance and physical and mental health clinical services provided to KS students. It is comprised of the Health Services, Behavioral Health, and Athletic Health departments that support the three campuses, 30 preschools, and summer programs. The Mālama Ola division contributes to E Ola! by supporting child well-being, which helps optimize student learning.
Health Services staff are responsible for performing the medical clearance for all student and are available to provide clinical assessments, first aid, and initial urgent/emergency care to students located on a KS campus.
Athletic Health staff provide primary care, injury and illness prevention, wellness promotion and education, emergent care, examination and clinical diagnosis, therapeutic intervention and rehabilitation for student-athletes.
Behavioral Health staff are available to provide crisis risk assessments, depression risk screenings, mental health counseling, as well as assist students and their caregivers in the coordination of outpatient mental health services for students.

In non-emergency situations, students can utilize the Hi‘ikua Line to report any issues they are not comfortable bringing to their principals or counselors.
